Digital care transition for hospitals

Pflege Patienten supports the search for follow-up care – it replaces neither your discharge management nor the hospital information system. Your team posts the scope of care required, care providers in the service area are informed and report back on whether they can accept the case. Posting is free of charge.

An example from discharge planning

A patient is to be discharged in nine days. Medical treatment care has been arranged with a known service; a provider is still missing for personal care and domestic assistance. The social service posts exactly these two open areas.

One care request instead of individual calls

You do not select any providers and do not write to anyone individually. All registered home care services and care providers whose service area covers the place of residence and whose location profile matches the care need are informed. The care providers decide for themselves whether they can offer care.

The case page additionally shows providers from the region that are not registered on Pflege Patienten, taken from a provider directory and marked ‘not on Pflege Patienten’. You call them yourself and record the call in the case. Providers can add themselves to the directory or object to their entry.

Your cases in one place

The social service sees all posted cases with their status – drafts, ongoing and ended referrals. The screenshot shows the German user interface.

Teamwork

Every member of staff in your social service works with their own access. You manage your team with roles and areas, set up deputies and keep track of who is responsible for each case. Sign-in with a second factor (app code via TOTP, plus recovery codes) is mandatory for hospitals.

Documentation

Pflege Patienten shows the status of the responses in each case. Your team sees which care providers were notified – with date and result. You download the record of the search for care as a PDF. This can also support the documentation if transitional care in hospital (Übergangspflege, Section 39e SGB V) becomes necessary. The platform does not replace your own discharge and record documentation.

Data processing

For your hospital, Pflege Patienten processes the case data as a processor on the basis of a data processing agreement (Auftragsverarbeitungsvertrag, AVV). You obtain the patient’s consent to the referral; in the portal you confirm that it has been given. Care providers initially see only the details for the referral and your work contact (role, ward, shared mailbox), without names and address. You pass on the name, address and date of birth directly to the selected care provider after the selection – not through the portal.

How the introduction works

1. Name a contact person

A responsible person in the social service coordinates the launch.

2. Set up access

The platform runs in the browser. No installation or interface is required. Sign-in is protected by a second factor.

3. Get to know the process

We will show you the process in a short call using invented sample cases.

4. Agree a start date

You decide from when your team posts real care needs.

What Pflege Patienten does not replace

Organising further treatment, medical documentation and the necessary handovers remain part of your existing care practice. Pflege Patienten does not replace a hospital information system.

Questions from hospitals

No. Pflege Patienten is intended for the referral step and runs in the browser. It does not replace a hospital information system, and no interface is required. Further integrations are not currently available.

Yes, with personal access: each person signs in with their own access. You manage your team with roles and areas and set up deputies. Sign-in with a second factor is mandatory for hospitals. Please do not use shared sign-in details.

That depends on the role, the permissions and the status of the agreement. Matching care providers first see the details for the referral – including type of care, place and approximate distance, services with times, care level (Pflegegrad), age group, diagnoses, short description and the referring organisation with its work contact – but no names and no address. Only the home care service or care provider that is selected receives the telephone number and email address of private individuals. We cannot promise that nobody could ever identify the person concerned; details are given on the page Data protection and security.
