Seek care

Place patients without writing to every care provider individually.

Post the scope of care required once. Pflege Patienten informs the registered home care services and care providers in the service area. You receive responses and agree the next steps with interested care providers. There are no platform fees for posting. Are you a private individual looking for care for yourself or a relative? Then the page for private individuals is the right place to start.

Who posting is intended for

Private individuals and relatives

You are looking for care for yourself or – as an authorised representative – for a relative. Posting is free of charge for this too.

For private individuals

Hospital and social services

Follow-up care after discharge is requested regionally – without a telephone list.

More for hospitals

Medical practice

An identified care need is passed on in a structured way instead of searching for individual services.

Advisory service and care support centre

You help people seeking advice to describe the care need in a way that care providers can assess it.

Care provider passing on a care need

Your home care service cannot cover one area? Post the open part free of charge.

What you gain

Post once

The care need is described once and not explained anew for every care provider.

Notify regionally

All registered home care services and care providers in the service area are informed – without selecting individual providers.

Responses in the case

Expressions of interest and questions come together in the respective care need.

Partial care

Care providers can express interest specifically in individual service categories.

Even if only part of the care is missing

Are you a care provider yourself and cannot cover one area? You can still post the open care need free of charge. A paid location subscription only concerns use as a care provider accepting cases.

Example

A home care service posts an open part

  • Personal care (by the service itself)already covered
  • Medical treatment care at weekendsopen
Invented example. Posting is free of charge – also for care providers with a location subscription.

What to have ready for a case

  • the postcode and the town where the care is to take place,
  • the desired start,
  • the services required and one to three types of care,
  • a work contact for your organisation – role, ward or shared mailbox, no private details,
  • the consent of the person concerned to the referral. For organisations, you hold the consent; in the portal you confirm that it has been given.

You do not upload any documents or records for the referral.

How to post a care need

The simple version has three pages: ‘What is needed?’, ‘Where, from when and for whom?’ and ‘Review and publish’. ‘Detailed information’ switches to the extended version. Until it is published, the care need remains a draft; care providers only see it afterwards. You then see all your cases together with their status. The screenshots show the German user interface.

Working with your team

Each person works with their own access. In the portal you manage your team with roles and areas and set up deputies. Sign-in with a second factor (app code via TOTP, plus recovery codes) is mandatory for hospitals; other organisations can make it mandatory for their team.

Data protection in brief

Care providers first see only the details for the referral and your work contact, without the name and address of the person concerned. You pass these details on directly to the care provider after your selection – not through the portal. For your organisation, Pflege Patienten processes the case data as a processor.

Questions about seeking care

Care needs can be posted by hospitals and their social services, medical practices, advisory services and care support centres, care providers passing on a care need, and private individuals and their authorised representatives. Registration takes place in the portal – as an organisation or as a private individual; the page ‘Registration’ describes the steps.

There is no platform fee for posting a care need. This says nothing about how the care services themselves are funded – that depends on your long-term care insurance fund (Pflegekasse) or statutory health insurance fund, or on your agreement with the care provider.

Yes. A care provider can, for example, post care that is already partly organised if further services are missing. Posting free of charge is separate from the paid location subscription for accepting cases.

No. The contact form is only intended for general questions, requests for a presentation and data protection requests. Please do not send any diagnoses, prescriptions or other patient details there. Care needs are posted exclusively in the protected portal.
