For home care services and care facilities
You can no longer care for a patient? Find a suitable follow-up solution.
Whether staff shortages, a round that no longer fits or a service your provider does not cover: post the open care need free of charge. Matching providers in the region are informed.
Find new patients – and organise follow-up care
Pflege Patienten works in both directions. You have capacity? Receive matching care requests. You can no longer take on an existing care arrangement, or only in part? Post the open need and find a suitable provider.
One location can do both – with the same access. If a person in need of care has to change their home care service for part or all of the care, follow-up care organised early helps everyone involved.

Typical situations
No capacity left, or care that no longer fits your service: passing the care on is then often the responsible solution – for example, when:
Staff are lacking
Staff shortages or a longer absence of an employee leave no capacity for all visits.
The round no longer fits
The times or extent of the care can no longer be fitted into your rounds.
The journey is too long
The place of care lies too far outside your service area.
The care has changed
The need has grown or changed so that you no longer cover it fully.
A service is no longer possible
You can no longer provide a particular service yourself.
A new type of service is added
A service is needed that your provider does not offer, such as medical treatment care or household help.
A service is to be reorganised
A single service category is to come from another provider in future.
Partial care: refer individual services onwards too
You do not have to pass on the whole care. You can also post individual open service categories. Two invented examples:

- Personal carecovered
- Medical treatment carecovered

How it works in five steps
1. Choose the care need
In the portal you choose “Post a care need” and state whether the whole care or individual services are sought.
2. Post the case pseudonymised
You describe the need without name and address: type of care, place, start and the services needed.
3. Matching providers in the chosen area are informed
Registered home care services and care facilities whose service area covers the place of the need receive a notice.
4. An interested care provider responds
Providers express interest in the whole care or individual services. Interest is not yet a commitment.
5. Agree on and hand over the care
You agree on the handover. It is only agreed once both sides have confirmed it.
Data protection in brief
You post the case pseudonymised – without the name, address, date of birth or insurance number of the person concerned. When posting, you confirm that their consent has been given.
For organisations that post cases, Pflege Patienten processes the case data as a processor; the basis is a data processing agreement with your organisation.

What does it cost?
Posting is free of charge – registration, the entry in the provider directory and posting a care need.
The location subscription (€99 plus VAT per location per month) only concerns taking on care, that is, when your location itself takes on requests from others.

Only household help to refer onwards?
There is a separate page for help in the household. The background is the draft Long-Term Care Reorganisation Act (PNOG) – a draft, not yet passed.
