Out-of-hospital intensive care

Find out-of-hospital intensive care

Anyone who is permanently ventilated or needs intensive nursing monitoring depends on specialised care outside hospital too – at home, in a residential unit or in a care facility. Pflege Patienten helps you request this care.

Service area: 1–150 km around the location

What this page does – and what it does not

Whether out-of-hospital intensive care is needed and to what extent is decided by doctors; it is usually prescribed by a doctor. Pflege Patienten replaces neither this decision nor medical advice.

The platform helps with the next step: finding a provider who can take on the prescribed care.

Details that specialised providers need

Place of care

Postcode and town where the care is to take place – in the person's own home or in a facility.

Special requirements

Whether, for example, ventilation has to be managed, you state as a detail in the request.

Start

For example the planned day of discharge from hospital.

Contact person

Who can be reached for questions. A provider only sees your contact details after you have selected it.

Home care services and care providers can mark features such as ‘Intensive care / ventilation’ or ‘AKI Section 37c SGB V’ in their location profile as a requirement for requests. Which care fits in the individual case you clarify with the provider and the treating doctors.

The route to intensive care outside hospital

1. Clarify with the doctors

Discuss with the hospital or the doctor which care is needed and from when.

2. Post the request

In the portal you choose out-of-hospital intensive care and describe the requirements.

3. Check the responses

Specialised providers in your region whose offer fits the request express interest or ask questions.

4. Plan the transition

After your selection and confirmation by both sides, you coordinate the transition directly with the provider.

When the hospital is already searching

If the hospital's social services or discharge management are already arranging follow-up care, ask there whether a request is already running. A second request for the same care is then not necessary.

Other types of care

If no intensive nursing care is needed, you will find the other types of care here.

Questions about intensive care

No. Pflege Patienten is not an emergency or acute care service. In an emergency, call 112; outside surgery hours you can reach the medical on-call service on 116 117.

Yes, if you are allowed to represent them – for example with a lasting power of attorney (Vorsorgevollmacht), as their legal guardian (rechtliche Betreuung) or because they have expressly asked you to. Being related does not in itself entitle you to pass on another person's health details. If the person can still take part in the decision, involve them. This website cannot judge whether a power of attorney is sufficient in an individual case.

Registered home care services and care providers whose service area covers the place you entered and whose offer fits the request. They see the details needed for matching – such as the type of care, place and approximate distance, start, services required and care level – but no name, no address and no date of birth. A care provider only sees your telephone number and email address once you have selected it.

Yes. In the portal you edit your request, drop services that are no longer needed or end the request, for example once you have arranged the care elsewhere. If no takeover has been agreed for any service after 14 days, the request is closed as 'not placed'; on day 11 you receive an advance warning.
