Features
Features for referrers: seeking care
What hospitals, social services, practices, counselling services and private individuals can do in the portal – from posting a care need to the history of the case.
Referrers post a care need in the portal and keep track of it until the care has been agreed.
Posting a care need
- Create a case in a simple or a detailed view – with type of care, place, start, the services needed with times, care level, diagnoses and the care situation. You can save a draft at any time.
- Before publishing, see the case the way care providers see it, and confirm that the consent of the person concerned has been given.
- The portal does not ask for names, addresses, dates of birth or health insurance numbers; it checks free texts for such details.
Responses and choosing an offer
- The responses of care providers appear together on the case: interest with the services a care provider can take on, questions or refusals.
- Each request comes with a text conversation with the care provider.
- You choose an offer; individual service areas can also be placed with different care providers.
Overview and history
- The history of each case shows which care providers were informed and how they responded.
- The “Record of care search” summarises this for your documentation – as a PDF and as a machine-readable file.
- The cockpit shows teams that post cases which cases need attention right now.
- Reports show aggregated key figures of your organisation – no individual persons.
- If too few care providers respond, the case shows further providers nearby that do not take part in the portal; you record calls and requests on the case.
Team
- Each person works with an account of their own; the administrators of your organisation invite the team and assign the roles.
For private individuals
Private individuals look for care for themselves or a relative with a free account of their own. An assistant guides them step by step through the request; a display name is enough as a name. Only the care provider you choose receives your phone number and email address.
