What is Pflege Patienten?

How the platform passes a care need on to suitable care services and care providers in the region – and what it does not do.

Pflege Patienten is a platform for placing people in care. A care need is posted once and goes to the care services and care providers in the region whose offer matches. The responses arrive together in one place.

Who uses Pflege Patienten?

  • Referrers post a care need: hospitals and their social services, medical practices, counselling services and care services – and private individuals looking for care for themselves or a relative.
  • Care providers offer care: home care services and residential care providers, for example care homes, short-term care or day care.

Each location of a care provider decides itself whether it seeks care, offers care or both.

How a placement works

  1. A referrer posts the care need: type of care, postcode and town, start, the services needed and the details a care provider needs for its decision. Names, addresses and dates of birth are not entered.
  2. Pflege Patienten informs the care providers whose service area and offer match the need. There is no preselection by the platform and no matching by artificial intelligence.
  3. Care providers express interest and name the services they can take on – or ask a question or decline.
  4. The referrer chooses an offer, also for individual services. A takeover only applies once both sides have confirmed it. The parties then exchange name and address directly, not through the portal.

What Pflege Patienten does not do

  • The platform does not decide who provides the care – the referrer, the person concerned and the care provider decide.
  • It does not guarantee care: whether a care provider responds depends on the offer and capacity in the region.
  • It does not replace the discharge management of a hospital.
